This is the voter registration form that we send out in August
each year to every domestic property in the district. This form
collects the information we need to publish a revised electoral
register on 1 December.
Please read the notes on the back of the form before you fill it
in - some additional guidance is given below to help answer any
questions you may have.
Please note that the form is addressed to 'The
Occupier' as it needs to be dealt with by the current
occupiers of the property and not necessarily by the person or
persons registered there.
If you do not respond to this form you may not appear on the
revised register and you might lose your right to vote. Registering
for other services or paying Council Tax does not register you to
vote. You are required by law to give the information asked
for on this form.
Note 1 - the property address
This is where the address we hold for your property will appear,
if you notice any mistakes please correct them and return your form
by post.
Note 2 - registering by freephone or internet
This is the information you need to use to register by freephone or
internet. When prompted, enter both parts of the security code
printed in the box.
You can only use the internet and freephone services if there
are no changes to be made. You cannot use these
services if you need to add names or delete names from your
form.
You can use the internet and freephone services to choose
whether the names of electors in your household will appear on the
edited register and you can also apply for postal votes.
Note 3 - your previous address
If you have moved into this property since the last annual
review please use this box to put in your previous address (where
you were last registered). This is helpful in enabling the Council
to keep an accurate register of electors by making sure there are
no duplicate records.
Note 4 - details of people resident at this address who are 16
and over
If you need to add details please do so in block
capitals, this information has to be read and put into the
system by the Electoral Services team and if it is not clear the
details may not be interpreted correctly.
Surname, forename and initials - please list
anyone
eligible to be registered by ensuring their
surname is in the first column followed by their forename and any
initials in the second column.
16 and 17 year olds - this column should
include the date of birth of 16 and 17 year olds eligible to be
added. As soon as they reach their eighteenth birthday they
can vote. If their date of birth falls between 1 December and
30 November they should be included.
Edited Register - this column is for
registering whether or not you would like your name included on the
edited register. The edited register is available for anyone to
purchase and may be used for any reason. If you do not want your
name included on this register please tick this box against your
name. If a pre-printed 'Z' is there it means that you have
previously opted out of appearing on the edited register and you
are free to cross it out if you now want your name to appear on
it.
All eligible electors are included on the full register of
electors whether they have opted to be included on the edited
register or not and the full register is the register used for
election purposes.
Jury Service - this column is to let us
know whether you are aged 70 and over. A pre-printed
'X' in this column shows that you have previously indicated that
you are. People who are 70 and over are excluded from jury
service.
Nationality - all electors must state their
nationality, follow this link to see a full
list of the nationalities entitled to
register. If this column is not completed the form will be
returned to you for this information to be provided. On forms
where electors’ names are pre-printed the nationality they have
stated has been added by using the internationally recognised
country codes. Please feel free to amend this if it is
incorrect.
Postal Vote - the last column is for you to let
us know whether you would like to receive an application to vote by
post. If an 'A' is pre-printed in the box you already have a postal
vote, if a 'P' is pre-printed in the box you have a proxy vote in
place.
Strict rules apply to proxy voting on a permanent basis and are
only be granted on medical, educational or employment grounds and
the application must be supported by an attestation. Please
consider this if requesting an application to vote by proxy.
