The designated premises supervisor (DPS) is responsible for
authorising the sale or supply of alcohol at a premises. That
individual must hold a personal
licence and only one DPS may be named on a premises licence at
any one time.
If the responsibility of being DPS needs to be given to another
individual for any reason (if the current DPS moves away from the
premises, for example) an application to vary the DPS will be
required.
A variation can only be made by the holder of the premises
licence or solicitors acting on their behalf. The consent of
the person who is to become the new DPS must also be given.

Please note that application forms in Word format can
be filled in on your computer and printed off for submission but
completed forms must be signed and submitted to us as hard
copy.
If you want to fill in your application by hand, print out
either a PDF or Word version and complete it using
black ink and block capitals. This is important because
the form will be scanned.
The fee is £23.00 payable to Suffolk Coastal District
Council.
Send one copy of the completed application form and
the DPS consent to transfer form to the Council’s
Licensing Team and one copy of the completed forms to the police at
the address below:

The current premises licence must also be included with the
